Heeley Trust is a community charity working in Heeley and Gleadless.
They have a range of services.
Social Prescribing
Social prescribing is a way of supporting you to take care of your own health and wellbeing. Heeley Trust’s Social Prescribing Link Workers will get you connecting with the local services, activities, groups etc that will get you where you want to be. They will listen and focus on what matters to you and help you create a personalised plan for your own health and wellbeing.
Referrals: You may be referred by a professional e.g. a health care worker, like a GP or physio, or by contacting them yourself. They will get back to you by phone/text and book in a chat with a link worker.
Locations: You can access this service if you are an adult and registered as a patient with the practices in the Heeley Plus Primary Care Network:
- Carrfield Medical Centre
- Devonshire Green and Hanover Medical Centres
- Gleadless Medical Centre
- Heeley Green Surgery
- Sharrow Medical Centre
- The Mathews Practice – Belgrave and White Lane
- Veritas Health Centre
Other services they provide include:
- Nature Projects: Nature Natter is an all-outdoors, all-weather group for people to come and spend time in the woods, chat and do activities based on bush craft, nature connection and creativity.
- Digital Drop-in: Free internet access & digital skills support at Newfield Green Library and Heeley Green Community Centre. See their website for details of when these sessions happen.
- Live Well with Dementia Projects: Heeley Trust is part of the citywide People Keeping Well Partnership in Sheffield. Their dementia projects offer activities and groups for people living with Dementia and their carers, as well as offering signposting advice and additional support services.
