Health Inclusion Team is a specialist NHS team of Health Visitors, Nurses and a Family Support Worker who provide health support to children and families in Sheffield. They work with families with children aged 0 to 19 who are:

  • living in temporary accommodation
  • gypsies and travellers
  • victims of domestic abuse and trafficking
  • asylum seekers and refugees.

They work closely with Phoenix Futures and Abbeyfield Family Centres as well as other housing and charitable organisations in Sheffield. Their main aim is to support the health needs of you and your family.

You can refer yourself to their service if you are already in temporary accommodation, a hostel or refuge. They also accept referrals from other charitable and voluntary organisations, as well as health and social care professionals working in the city.

Asylum seeking families who need support must be registered at the Mulberry Practice GP surgery to be eligible for our support.
