Specialist Community Learning Disability Service offers support to people with learning disabilities and their carers in Sheffield.
The Specialist Community Learning Disability Service are a multi-disciplinary team. They offer comprehensive, holistic, health assessments, diagnosis, support and care for people aged 18 and above. They offer both standard and enhanced support to provide care based on a person’s specific needs.
Referrals are accepted from anyone, including service users and their families. To make a referral you need to fill in a referral form, which you can find on their website, and return it via email to [email protected] or by post to their address.
