Community Learning Disability Team is a specialist health service for people with learning disabilities and their carers in Sheffield.

Community Learning Disability Team are a multi-disciplinary team.  They offers comprehensive, holistic, specialist health assessments, diagnosis, support and care for people aged 18 and above.

Referrals are accepted from primary care (GPs), secondary care and social care services as well as from service users’ families and voluntary or private sector organisations. To make a referral you will need to fill in a referral form and return it via email to CLDTbusinesssupport@shsc.nhs.uk or by post to their address.

Forensic Outreach Liaison Service

The Forensic Outreach Liaison Service is a service that works alongside the Community Learning Disability Team. They supports people aged 18 years and above who have a learning disability or autism or both, who have come into contact with the criminal justice system.  You can find out more about the service by clicking here.
