If your mental health difficulties are more serious or complex, or you have not found primary care services helpful, you might be referred to the specialist mental health services within Sheffield Health and Social Care Foundation Trust (SHSC).

After an assessment, you may be allocated a care co-ordinator who will work with you to develop a care plan detailing the treatment and support you will receive and the anticipated benefits to you. Care co-ordinators are usually social workers, community psychiatric nurses, occupational therapists or other mental health professionals.

There are a number of specialist teams who work within SHSC, such as:

  • Assertive Outreach Team
  • Specialist Eating Disorders Service
  • Specialist Psychotherapy
  • Perinatal Mental Health
  • Continuing Needs Team
  • Early Intervention in Psychosis Team
  • Crisis Resolution Home Treatment Team.
