If your mental health difficulties are more serious or complex, or you have tried the help that is on offer in primary care and it has not helped, you might be referred to the specialist mental health services within Sheffield Health and Social Care Foundation Trust (SHSC). Here you may be given a care co-ordinator who will work with you to develop a care plan detailing the treatment and support you will receive and the anticipated benefits to you. Care co-ordinators are usually social workers, community psychiatric nurses, occupational therapists or other professionals who are also mental health specialists. There are a number of specialist teams who work within SHSC such as the Assertive Outreach Team, Specialist Eating Disorders Service, Specialist Psychotherapy, Perinatal Mental Health, Continuing Needs Team, Early Intervention in Psychosis Team and the Crisis Assessment Home Treatment Team. |
