CPA is the system by which the secondary mental health service (Sheffield Health and Social Care NHS Foundation Trust) delivers care and treatment. A principle of CPA is that service users and carers should be fully involved in the planning of their care. CPA is only used for people receiving support from secondary mental health services - it does not apply to those who are receiving care and treatment solely from primary care or voluntary organisations. Most people will be receiving ‘standard care’ (until October 2008 this was called ‘standard CPA’) and will often have only one professional involved. Their care plan will normally be received in the form of a letter. For those whose needs are more complex, and where there are several people involved, their care will be called ‘CPA’ (until October 2008 this was called ‘enhanced CPA’). For those receiving CPA, regular reviews will be held where the service user (and by agreement any carer) will meet with those involved to review their care plan. The care co-ordinator should prepare the service user for these reviews and discuss the most helpful way for them to be conducted. The service user is entitled to bring a friend or advocate to these meetings. Sheffield Mental Health Advocacy Service will be able to advise on this if appropriate. There is a CPA Review Code of Conduct leaflet which service users should receive. A service user may write an advance statement stating how they wish to be treated if they later become unwell and unable to express their wishes. There is a leaflet available, ‘Completing an Advance Statement’. The care co-ordinator will be able to provide this and advise further. Any written advance statement will be considered at the CPA Review. |
