Sheffield Assertive Outreach Team (SORT)
Sheffield Assertive Outreach Team is an assertive outreach service providing intensive community treatment and support to adults with severe and enduring mental health problems who have complex needs.
This usually means people who have a primary diagnosis of severe and enduring mental illness (for example schizophrenia or bipolar disorder) and a secondary diagnosis (for example substance misuse). Service users will usually have a long history of mental health problems and repeated hospital admissions.
The aim of the team is to improve the quality of life for service users to enable them to remain in the community wherever possible. The team is multi-disciplinary and consists of psychiatrists, social workers, community mental health nurses, psychologists, occupational therapists and support workers.
The majority of contacts take place in a service user's home or other community settings. They also provide some support to carers of clients.
Referrals are accepted from community mental health teams and other healthcare professionals working within Sheffield Health & Social Care.
