I need urgent help! What to do in a crisis

Community Dementia Support Service (CDSS)

CDSS provides a one-to-one day service for people with dementia and complex needs who, as a consequence of their behaviours, have difficulty interacting with others and therefore cannot access mainstream dementia day services.

Through individual care plans the team support service users to integrate into small groups. If this is successful, they are then transferred into mainstream social care day services.

Support is provided for between six and twelve weeks during which time regular assessments are carried out.

Referrals are accepted from Social Workers, Care Managers, Older Adult Community Mental Health Teams and GPs.
